Opportunity Information: Apply for CDC RFA DP 23 0002
School-Based Interventions to Promote Equity and Improve Health, Academic Achievement, and Well-Being of Students (CDC RFA DP 23-0002) is a discretionary, 5-year cooperative agreement from the U.S. Department of Health and Human Services, Centers for Disease Control and Prevention (CDC), specifically through CDC Healthy Schools within NCCDPHP. The opportunity is designed to protect and improve the health and overall well-being of school-age children, with a clear emphasis on students in underserved communities and communities that are disproportionately affected by poor health outcomes. The central idea is that healthier students are better positioned to succeed academically, attend school consistently, and thrive socially and emotionally, so the work is framed as both a public health and education partnership effort.
Under this funding, the CDC anticipates making roughly 16 awards, with the possibility of expanding to as many as 50 recipients if funding allows. The award ceiling is $425,000. Because it is structured as a cooperative agreement, recipients should expect substantial CDC involvement beyond typical grant monitoring, such as ongoing collaboration, guidance, and technical support around implementation and evaluation. Applications were due February 11, 2023, by 11:59 pm ET.
The program requires recipients to use the Whole School, Whole Community, Whole Child (WSCC) model, a framework that ties student learning and health together by coordinating policies, environments, and services across the school and its surrounding community. In practical terms, recipients are expected to build and strengthen partnerships that support school health programming, and then use those partnerships to spread and apply CDC school health guidelines, tools, and evidence-informed resources. A major mechanism for doing that work is professional development and technical assistance for education agencies, schools, and related partners, paired with structured action planning so that improvements are not one-off activities but are embedded into local systems.
Implementation is focused on three main areas: physical activity, school nutrition, and school health services. Recipients will support state and local education agencies, individual schools, and out-of-school time settings, recognizing that student health behaviors and supports extend beyond the traditional school day. Activities may include helping organizations adopt or strengthen policies, standardize practices, select programs aligned with CDC guidance, and develop the capacity of staff and partners to implement changes consistently over time.
The expected outcomes are specific and measurable. First, the project aims to increase the number of schools that develop an action plan grounded in completion of the CDC School Health Index, which is used to assess school health policies and practices and to guide improvement planning. Second, recipients are expected to increase implementation of Comprehensive School Health Activity Programs, reflecting a move toward coordinated, schoolwide approaches rather than isolated initiatives. Third, the work targets stronger school nutrition and food security policies and practices, addressing both the quality of nutrition environments and the reality that some students face inconsistent access to healthy food. Fourth, recipients should expand school-based health services that support students with chronic health conditions, which can include improving how schools identify needs, coordinate care, and provide accommodations and supports that help students manage conditions while staying engaged in learning. Fifth, the grant aims to produce student-level improvements, including increased physical activity, healthier dietary behaviors, and stronger self-management of chronic conditions, which together are intended to reduce health disparities and improve day-to-day functioning and long-term outcomes.
Eligibility is broad and includes state, county, city or township governments; special district governments; independent school districts; public and private institutions of higher education; federally recognized tribal governments and other tribal organizations; public housing authorities/Indian housing authorities; nonprofit organizations with or without 501(c)(3) status; for-profit organizations (other than small businesses) and small businesses; and other entities as allowed by the notice. This wide eligibility reflects the program’s emphasis on cross-sector partnerships and the reality that strong school health work often involves education agencies, public health agencies, community organizations, healthcare partners, and others working together.
Overall, this opportunity funds structured, partnership-driven school health improvements using the WSCC model, with the goal of advancing equity and producing concrete changes in school planning, physical activity programming, nutrition and food security practices, and health services that help students, particularly those in underserved communities, be healthier and more ready to learn.Apply for CDC RFA DP 23 0002
